Four new drugs will change prostate cancer care, Colorado expert ...

After a decade and a half of near stagnation, four new drugs could help make advanced prostate cancer a chronic illness instead of a terminal disease, a leading Colorado prostate cancer expert says.

?It?s not just chemotherapy. The drugs have different and innovative methods of action. One is a bone protective agent; another?s a more effective hormone agent; another is radiotherapy; and the final one is the first drug tested for cancer immunotherapy,? says E. David Crawford, MD, investigator at the University of Colorado Cancer Center and head of urologic oncology at University of Colorado Hospital.

?Even without the addition of any more drugs, we may now have the tools that in combination will allow us to extend the survival prognosis of a prostate cancer patient long enough to make prostate cancer a disease a patient is more likely to die with than from,? Crawford says.

Along with Thomas Flaig, MD, Crawford describes these advances in prostate cancer treatment in a recent review for the journal Oncology.

First is the drug Denosumab, which Crawford says, ?has three uses in protecting the bones of prostate cancer patients.? It can prevent bone fractures in patients with existing bone metastasis; it can prevent osteoporosis in patients whose calcium is depleted as a side-effect of hormone therapy; and (pending FDA approval) it has been shown to hold off the occurrence of bone metastasis for an average of four months in patients whose spiking PSA scores predict the likely onset of bone involvement.

Second is the drug Alpharadin, which is one of a novel and exciting class of ?radiopharmaceuticals? ? drugs that emit radiation and allow doctors to precisely deliver radiation to tumor sites. In the case of Alpharadin, it emits alpha rather than beta particles, which allows more precise tumor targeting of bone metastasis sites with less collateral damage to surrounding bone marrow.

Third, the drug Prostvac is the first ?immunotherapy? drug used for the treatment of cancer. The drug acts like a vaccine, priming the immune system to recognize and thus fight against prostate cancer cells. In a phase II clinical trial of 125 patients, the drug extended the median survival time from 16.6 to 25.1 months.

Finally, the drug Abiraterone Acetate completely suppresses the body?s ability to make testosterone, which many prostate cancers need to grow (as opposed to previous drugs, which hoped to out-compete testosterone with estrogen, or imperfectly controlled testosterone production).

Crawford notes that these drugs are being approved for use only after more established therapies have failed and hopes that in coming years science may accelerate the use of these drugs to first-line therapies.

States, banks reach foreclosure-abuse settlement (AP)

WASHINGTON ? U.S. states have reached a $25 billion deal with the nation's biggest mortgage lenders over foreclosure abuses that occurred after the housing bubble burst.

Federal and state officials announced the deal Thursday. It is the biggest settlement involving a single industry since a 1998 multistate tobacco deal.

Under the agreement, five major banks ? Bank of America, JPMorgan Chase, Wells Fargo, Citigroup and Ally Financial ? will reduce loans for nearly 1 million households. They will also send checks of $2,000 to about 750,000 Americans who were improperly foreclosed upon. The banks will have three years to fulfill the terms of the deal.

All but one of the 50 states agreed to the deal. Oklahoma, the lone holdout, will receive no money.

The conditions will be overseen by Joseph A. Smith Jr., North Carolina's banking commissioner. Lenders that violate the deal could face $1 million penalties per violation and up to $5 million for repeat violators.

The settlement ends a painful chapter that emerged from the financial crisis, when home values sank and millions edged toward foreclosure. Many companies processed foreclosures without verifying documents. Some employees signed papers they hadn't read or used fake signatures to speed foreclosures ? an action known as robo-signing.

Under the deal, 49 states said they won't pursue civil charges related to these types of abuses. Homeowners can still sue lenders in civil court on their own, and federal and state authorities can pursue criminal charges.

"There were many small wrongs that were done here," said U.S. Housing and Urban Development Secretary Shaun Donovan. "This does not resolve everything. We will be aggressive about going after claims elsewhere."

Bank of America will pay the most to borrowers as part of the deal ? nearly $8.6 billion. Wells Fargo will pay about $4.3 billion, JPMorgan Chase will pay roughly $4.2 billion, Citigroup will pay about $1.8 billion and Ally Financial will pay $200 million. This does not include $5.5 billion in federal and state payments.

The deal also ends a separate investigation into Bank of America and Countrywide for inflating appraisals of loans from 2003 through most of 2009. Bank of America acquired Countrywide in 2008.

"The settlement includes far reaching relief that will help many of our customers and complement our already extensive efforts to improve our borrower assistance efforts and servicing processes," JPMorgan Chase said in a statement.

The banks and U.S. state attorneys general agreed to the deal late Wednesday after 16 months of contentious negotiations.

New York and California came on board late Wednesday. California has more than 2 million "underwater" borrowers, whose homes are worth less than their mortgages. New York has some 118,000 homeowners who are underwater.

In addition to the payments and mortgage reductions, the deal promises to reshape long-standing mortgage lending guidelines. It will make it easier for those at risk of foreclosure to make their payments and keep their homes.

Those who lost their homes to foreclosure are unlikely to get their homes back or benefit much financially from the settlement.

The settlement would apply only to privately held mortgages issued from 2008 through 2011. Banks own about half of all U.S. mortgages ? roughly 30 million loans. Those owned by mortgage giants Fannie Mae and Freddie Mac are not covered by the deal.

Some critics say the proposed deal doesn't go far enough. They have argued for a thorough investigation of potentially illegal foreclosure practices before a settlement is hammered out.

Under the deal:

? Roughly $1.5 billion for direct payouts, in the form of $2,000 checks, for about 750,000 Americans who were unfairly or improperly foreclosed upon; another $3.5 billion will go directly to states.

? At least $10 billion for reducing mortgage amounts.

? Up to $7 billion for other state homeowner programs.

? At least $3 billion for refinancing loans for homeowners who are current on their mortgage payments but who are underwater.

Report: Facebook to start mobile ads by March

Facebook, the social network giant, is set to begin showing advertisements to users on mobile devices before its $5 billion initial public offering, the Financial Times reported on Monday.

Facebook has already discussed proposals with unidentified advertising agencies for displaying what it calls "featured stories" in the news feed seen by users on the site in an effort to tap a new source of revenue, the newspaper cited several people familiar with the plans as saying.

Facebook mobile device users are likely to see advertisements in March ahead of its initial public offering, which is expected in May, according to the FT.

Maine girl bouncing back after 6-organ transplant (AP)

HOLLIS, Maine ? A 9-year-old Maine girl is home from a Boston hospital healthy, active and with high hopes ? and a new stomach, liver, spleen, small intestine, pancreas, and part of an esophagus to replace the ones that were being choked by a huge tumor.

It's believed to be the first-ever transplant of an esophagus and the largest number of organs transplanted at one time in New England.

Spunky and bright-eyed as she scampered around her family's farmhouse outside Portland, Alannah Shevenell said Thursday that she's glad to be feeling well again and able to go sledding, make a snowman, work on her scrapbooks and give her grandmother a little good-humored sass.

The best part, though? "Being home," she said. "Just being home."

It was 2008 when Alannah, then 5, began running a fever and losing weight while her belly swelled. Doctors discovered the tumor that year and twice attempted to remove it, as it made its way like octopus legs from organ to organ. But it was difficult to access what turned out to be a rare form of sarcoma, said Debi Skolas, Alannah's grandmother, and chemotherapy didn't do the trick, either.

All the time, the growth ? known as an inflammatory myofibroblastic tumor ? continued to grow in her abdomen, causing pain, making it hard to eat and swelling her up with fluid. Surgery was the last resort to save her life, and Alannah spent more than a year on a waiting list for the organs, said Dr. Heung Bae Kim, the lead surgeon on the procedure at Children's Hospital Boston.

The family was told there was a 50 percent chance Alannah wouldn't survive the procedure. But without it, she had no chance whatsoever.

Things were more tense than celebratory in October when doctors prepared to remove the growth and the organs in one fell swoop and replace them with organs transplanted in one tangled piece from another child of similar size.

The hardest part was taking out her organs and the tumor, Kim said, calling it a difficult operation with lots of blood loss.

"It's probably one of the most extensive tumor removals ever done," the surgeon said.

Dr. Allan Kirk, professor of surgery at Emory University in Atlanta and the editor-in-chief of The American Journal of Transplantation, said no other esophageal transplant has been reported in medical literature.

After the surgery, Alannah spent three more months at the hospital, with her grandmother sleeping every night in a lounge chair by her bed. She battled infections and complications from the surgery before finally being given the OK to leave.

She arrived back home Wednesday in the 192-year-old house on a country road where she lives with her grandmother and grandfather, Jamie Skolas, in Hollis, a town of 4,500 residents about 20 miles west of Portland.

But just because she's home doesn't mean she's out of the woods. Alannah has to take nine medications each day, some two, three or four times. Her grandparents have to precisely measure what goes in and comes out of her body, and check her blood sugar.

She has an ostomy pouch and feeding tube attached to her for nutrition as she slowly gets used to eating again. Scars from her surgeries look like a roadmap on her stomach. A tutor comes to the home 20 hours a week for her schooling.

Her immune system is so weak that she can't go to places with large numbers of other people, such as school, church or a mall. She can't eat raw vegetables or fruits unless they have thick skins because of concerns over germs, and she'll never be able to swim in a lake because of the bacteria. The Skolases installed ultraviolet lights in their heating ducts to kill mold, mildew and bacteria that might sicken Alannah.

Alannah is aware of her limitations and what she's been through. "Don't even ask," she says when the subject of the medical costs, which have been covered by MaineCare ? Maine's version of Medicaid ? come up.

She's talkative and enjoys bantering with her grandparents.

"Grammy, you're not always right," she said to end a conversation.

The Skolases, who took Alannah in several years ago but declined to discuss the whereabouts of her parents, have made sacrifices for her through the years. Their hand-crafted-furniture business has suffered, with Debi devoting her time to care for Alannah, and the couple has dipped into retirement savings to make ends meet.

Friends have organized a fundraiser to help raise money to offset the costs.

More than anything, though, the family is thankful for the girl's second chance at life and to the family that went through the pain of losing a child and before deciding to donate the organs to help Alannah.

"That was a courageous decision," Debi Skolas said. "I still cry when I think about it."

First impressions stay with you. Take our first look at the Nintendo 3DS, for example. Our first thought? "Hey, where's the second analog stick?" Multiple Sclerosis Blog - Everyday Health

Of the many, many things that the Life With MS Blog has become, it is an open venue of back-and-forth, give-and-take. Begging an indulgence, I?m going to ask that you give so that I can take and then give back again.

I am in the process of editing what is sometimes called a ?speaker?s resume video.? It?s a short collage of snippets from addresses I?ve given over the past few years. I have been asked ? with increasing frequency ? to speak at events and many event organizers have asked for samples of my ?work.? Most of these events are multiple sclerosis-related, so it?s pretty easy to cobble together a story on the fly.

A 20-60 minute speech, however, calls for some planning (some).

I?ve sat riveted by a number of speakers on the topic of MS. I?m sure you have as well. At least as often, however, I?ve sat through such addresses wondering where the point lay, if the speaker knew, or if there were even a point at all.

It is customary to ask the body organizing the event what they might like me to speak about or, and more to the point, what the audience would want to hear. It occurred to me that our blog community is a much better place to ask the question.

I?ve heard enough ?poor me? speeches about MS and other conditions. Contrarily, I have also been had so much sunshine and rainbows blown up my kilt by some speakers that I wonder if I?m supposed to be happy and feel privileged to have this stupid disease. There must, I figure, be a place in between where a good and compelling story can be told without purposefully tugging on heart strings or skipping across the stage in pigtails.

Another reason I felt this a good place is that you are much of the reason I?m asked to speak in the first place. The attention you afford this community has made us a focal point for many seeking information about MS. Your stories are as much a part of my life with MS as my own. I figured you might have a few shareable anecdotes (comic and tragic) in your years with multiple sclerosis as well.

Another factor to consider is the others in these audiences. Many times our family and friends attend, and I don?t want to even think about the number of times I?ve heard someone in the crowd say, ?See, she can climb mountains with her MS. You must be lazy,? or something similar at an MS gathering.

What would you like your friends/family to hear were they to attend an event with a speaker about MS?

You know that I ask these questions because I value our group experience and see us all as experts at living with MS. This is part of that life as well, and I see good reason to ask you what you think.

Mommy Bloggers' Flawed Take on Anti-Obesity Ads (ContributorNetwork)

COMMENTARY | According to HLN, the newest and most vocal critics of Children's Healthcare of Atlanta's anti-obesity ad campaign are a group known as "mommy bloggers." These angry mothers feel the controversial ads, which depict obese children as unhappy and suffering from medical maladies, do more harm than good by shaming children instead of encouraging them. Critics argue that shaming tactics only lead to greater tendencies to overeat and can lead to higher numbers of eating disorders.

Shaming is a key point in the volleys of arguments.

While shaming can be horrid and abusive, I worry we've become a society so indulgent and fearful of shame that we are woefully lax and can hardly motivate or discipline our youth. We want to only use the carrot and never the stick. The problem for kids: The stick eventually finds you.

The stick is called the real world. Real life. Life beyond childhood and school-age indulgences.

While it hurts to think we might need to shame an overweight child, are we so naive as to think his or her first boss will be as indulgent as the parents or schoolteachers of yesteryear? Will a callous public be as politically correct and caring when the overweight person is an adult rather than a child?

As a young adult I've seen the differences between the real world I live in today and can compare them to the comparative ease of my high school and college days, still relatively fresh in my memory. The real world rarely gives second chances. The real world expects performance and results.

As a high school teacher I marvel at the apathy of many students and try to explain to them that, in a few short years, they will be studying or laboring under far different circumstances - for bosses and professors who do not give the second chances we K-12 teachers provide.

The real world does not hesitate to use shame or the proverbial stick. It can be a tough world, as I'm finding out while figuring a budget and looking at staggering real estate prices in a local market saturated with oil money. While it may be cruel to shame a child, what about a teen who is approaching sixteen and is only a few years away from adulthood?

We need to get results on our war on obesity. Fast.

CA Supreme Court reinstates murder conviction (AP)

SAN DIEGO ? The California Supreme Court on Monday reinstated a man's second-degree murder conviction for killing a well-known San Diego surfer, overturning a 2010 ruling by a state appeals court that had reduced it to voluntary manslaughter.

The state's highest court said it disagreed with the decision by the 4th District Court of Appeals that cited insufficient evidence of implied malice by Seth Cravens when he delivered the single fatal punch to 24-year-old Emery Kauanui in 2007.

Cravens was found guilty in November 2009 of second-degree murder and sentenced to 20 year to life in prison. The court ruling Monday means he will continue to serve that sentence.

If the voluntary manslaughter conviction had stuck, he could have faced a maximum of 16 years in prison.

Nicknamed the "Flying Hawaiian," Kauanui was a fixture at San Diego's Windansea Beach, where his favorite surf break is now called "Emery's Left."

Prosecutors said Cravens and four other men had gone to the La Jolla house of the surfer's mother to retaliate after Kauanui accidentally spilled beer on one of the men earlier in the evening at a bar.

After a group attack on Kauanui, Cravens delivered the punch to his head that prosecutors said fractured his skull. He fell then died at a hospital four days later.

"As the jury found, it was an extremely powerful blow to the head calculated to catch the impaired victim off guard, without any opportunity for the victim to protect his head, and thereby deliver the victim directly and rapidly at his most vulnerable to a most unforgiving surface," Justice Marvin Baxter wrote in his explanation of the Supreme Court decision.

There was one dissenting opinion. Associate Justice Joyce Kennard said she agreed with the lower court ruling that it is hard to prove Cravens knew his punch would kill Kauanui.

Kennard pointed out that Cravens punched the surfer with his less dominant left hand and "therefore had less reason to suspect that the blow would endanger Kauanui's life." Cravens also had delivered unexpected punches in past fights in which no one died.

District Attorney Bonnie Dumanis applauded the Supreme Court ruling, saying it "holds a murderer accountable for his crime and restores justice for Emery Kauanui, his family and friends."
